Library Registration Form

Registration Form for Library Services: Library Privileges and Off-Campus Access to Library subscriptions.

By completing this form, you are registering for an account in our library catalog. This account can be used to check out books and access online library resources including full-text electronic journals and books from outside the Center's network. An email with more information will be sent to you in 7-10 working days.

To be eligible for a library account, you must be a Fred Hutch employee, student, official affiliate investigator or an affiliate employee. If you are unsure about your eligibility status, please contact library staff for clarification.

Affiliate investigators or affiliate employees: Affiliates must fill out the HR Non-employee action form (restricted access). Affiliates should have their Fred Hutch sponsor print out and sign this form and send it to the Human Resources department for processing. Once processing is completed, HR will email you confirmation - please forward that confirmation to us at library@fredhutch.org or submit this form at this time. The library will then create an account and email your account details.
